Please attach a drawing or sketch of the project you are requesting. In most cases a drawing is required. If we do not receive a drawing attachment the approval process will be delayed.

Pursuant to the policies of South Shore Regional School District I request and authorize the students and instructors from the designated vocational areas to perform the work outlined in this requisition. This work request is made with full recognition that the curriculum, student learning priorities, school calendar limitations, and the timelines set by the district govern the acceptability of this project.

I also understand that the South Shore Regional School District and its employees are not liable for any loss or damage to personal, corporate or municipal property left for repairs or alterations. I further understand that there will be no time limit for the work to be completed and I accept all aspects of pickup and transfer of property, if applicable. I am also aware that should this project be accepted, a service fee may be required to be paid to the South Shore Regional School District.
