The purpose of this subcontractor evaluation form is to assess the performance of subcontractors on FCI Constructors" projects. The evaluation ratings will be used to compile a pre-qualified list based on job site performance. Subcontractors solicited for future work will be selected from this pre-qualified list. Use a 1 through 5 performance rating system, with 1=Poor, 2=Marginal, 3=Satisfactory, 4=Good, and 5=Excellent. The overall ranking will then be calculated for you. An overall ranking of 3 is required to make the pre-qualified list. Submitted evaluation forms will be shared with the Project Manager for comment, and then will be subsequently reviewed by procurement staff.
