Release and Waiver of Liability
• I understand that I will be actively participating in TRX Suspension Training and there is consequently some risk of injury. • I am responsible to judge my own capabilities and to move at a safe level for my own body. • I understand that it is my responsibility to ascertain that there is no medical reason to prevent my safe participation in this program. • I acknowledge that it is my responsibility to inform the instructor when I begin class of any injuries or condition(s) that I have. • I understand that the instructor(s) may provide me with gentle, hands-on adjustments and by signing this waiver I give my informed consent to these adjustments. • I, for myself, respective heirs, administrators and successors, intending to be legally bound, hereby release and indemnify YOGA ON YORK, LLC, the owners, and all instructors from and against all claims liabilities, damages or causes of action arising out of or in connection with my participation in the classes, without limitation.
Sessions non-refundable. 1 Free Class Make-Up pass will be given for one missed class per session and can be used for any regular YOY drop-in yoga class or TRX class.
