Is this your first dog? Does your budget allow for routine yearly exams and any emergencies that might come up?

What kinds of activities will you and your dog do together? If you have kids, what are their ages, and what role will the dog have in their lives?

If you have other pets, how will you make sure they all get along?

Are there behaviors or training concerns that might be ‘deal breakers’ for you? How will you handle any hiccups during the settling in process? How will you handle potty accidents, chewing, barking, etc?

Describe your available yard and your dog’s outdoor routine: Is your yard fenced? Are you in town or in the country? Are you on a busy street? Do you have neighbors with dogs that might interact with your dog? Do you have a tie out or plan to walk your dog frequently?

What brands of food do you like to use? Do you have a set feeding routine for your pets?

Where will you keep your dog at night or when you're not home? Do you have a plan to keep the dog from being naughty when it’s alone?

Please be aware that the Green Lake Area Animal Shelter is available to help with ANY concerns or issues that come up after you take your new pet home. Phone calls and emails are welcome, and home training visits are available on request. All of this is at no charge to you. We hope you will see us as your primary resource and call us anytime! In the event that your adoption is unsuccessful, please be aware that our adoption agreement specifically states that the cat must be returned to us. Exceptions to this requirement are at the discretion of the Shelter Manager. Thank you!

By submitting this application, I certify that the information I have given is true, and I recognize that any misrepresentation of the facts may result in my losing the privilege of adopting a pet. I understand that Green Lake Area Animal Shelter, Ltd. has the right to deny my request to adopt an animal. I understand that this information could be made available to other shelters.
