IMPORTANT NOTE REGARDING INSURANCE: All Above the Clouds pilots in command, whether aircraft owner or renter, are required to carry their own insurance policy. Pilots are asked to provide a certificate of insurance from the underwriter evidencing liability coverage. The certificate should name Above the Clouds, its directors and volunteers as additional insureds, but only with respect to operations by you. There should be no additional cost to you. Please send a photocopy of the front page of your policy showing you as the named and insured in order to complete this application.

By submitting this application, I hereby affirm that all information I have provided with this form is accurate, and that I agree to abide by all applicable Federal Aviation Regulations in the conduct of a flight and to provide a Dream Flight in an "airworthy" aircraft as defined by the Federal Aviation Regulations.
