All coaches must agree to the Acknowledgement statement below.  HEAD COACHES must also indicate affirmation and agreement.

ACKNOWLEDGEMENT

I commit to the Lord and to the Upward Leadership Team that I will serve as a coach for the entirety of the season unless I am prevented from doing so by family emergency or personal injury.  I understand that any negative personal habits, such as smoking, swearing, alcohol/drug abuse, immorality, pornography, etc, will hinder my ability to coach effectively and may negatively affect the spiritual development of the players on my team.  Understanding that these children will be looking up to me, I commit to setting a worthy behavioral example for them to emulate.  I acknowledge that failure to keep this commitment to living above reproach may be cause for the Leadership Team to remove me as a coach.  I also give South Rock Christian Church permission to do a personal background check on me.

Using John 3:16 "For God so loved the world that He gave His only Son, that whoever believes in Him should not perish but have eternal life." as a reference, do you believe in God’s Son, the Lord Jesus Christ?

Statement of Faith and Practice 

1.The Bible is the only inspired word of God, and it is complete, inerrant (in its original texts), and eternal.

2.God, in His Word, represents Himself as unique and three persons in one:  Father, Son, and Holy Spirit.

3.Jesus Christ, the one and only Son, became flesh by being born to a virgin, lived a perfect life, and was crucified as the Lamb of God to take away the sins of the world.  He was buried, rose from the dead, ascended to heaven, and is seated at the right hand of the Father.  He will return for His people.

4.All people are sinners.  A person must believe in Jesus Christ to be forgiven and have eternal life.

5.According to God's Word and the Kansas Constitution, marriage is only to be between one man & one woman.
