CHAMPION LIFE CENTRE (GUELPH)
ANNUAL FAMILY CAMP
WAIVER & RELEASE FORM
In consideration for participating as a camper/volunteer/visitor, and/or for consenting to my chid(ren)’s participation at the Champion Life Centre (Guelph)'s Annual Family Camp, I hereby freely and voluntarily, without duress, execute this Agreement under the following terms:
1. Waiver and Release. I, the camper/volunteer/visitor, and/or Parent/Guardian of Minor camper/participant/visitor, release and forever discharge and hold harmless Champion Life Centre and their respective officers, employees, volunteers, directors, agents, affiliates, successors and assigns from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from my participation in the Annual Family Camp of Champion Life Centre (Guelph). I understand and acknowledge that this Waiver and Release discharges Champion Life Centre from any liability or claim that I, the camper/volunteer/visitor, and/or Parent/Guardian of Minor camper/participant/visitor may have against Champion Life Centre with respect to bodily injury, personal injury, illness, death or property damage that may result from my activities and time spent at the Annual Family Camp. I also understand that Champion Life Centre does not assume any responsibility for or obligation to provide financial assistance or other assistance to me and to my child(ren), including but not limited to medical, health or disability insurance, in the event of injury, illness, death or property damage.
2. Insurance. I, the camper/volunteer/visitor, and/or Parent/Guardian of Minor camper/participant/visitor, understand that I expressly waive any such claim for compensation or liability on the part of Champion Life Centre beyond what may be offered voluntarily and freely by a representative of Champion LIfe Centre, in its sole discretion, in the event of such injury or medical expense.
3. Medical Treatment. I hereby release and forever discharge Champion Life Centre from any claim whatsoever which arises or may hereafter arise on account of any first-aid treatment or other medical services rendered to me and/or my child(ren) in connection with an emergency during my/our time with Champion Life Centre (Guelph)'s Annual Family Camp.
4. Assumption of the Risk and Indemnification. I understand that my time and/or my child(ren)'s time with Champion Life Centre (Guelph)'s Annual Family Camp may include activities that may be hazardous to me, including, but not limited to, construction activities, loading and unloading of heavy equipment and materials, other physical activities, such as but not limited to, swimming, games, running, and other sports-related activities, as well as local transportation to and from the campsite. I hereby expressly and specifically assume the risk of injury or harm in these activities and release Champion Life Centre from all liability or injury, illness, death, or property damage resulting from my participation and/or my child(ren)'s participation in these activities and my/our time at the Annual Family Camp.
5. Photographic/Video Release. I and/or my child(ren) grant and convey unto Champion Life Centre all right, title, and interest in any and all photographic images and video or audio recordings made by Champion Life Centre during my/our time at the Annual Family Camp, including but not limited to, any royalties, proceeds, or other benefits derived from such photographs or recordings.
I have read this Release, Waiver of Liability, Indemnity Agreement and Parental/Guardian Consent Agreement, and understand that I have signed it freely and voluntarily.
