  • If  answering "YES" to any of the following question;

    Please contact your regional director to arrange for approval of the site and/or trial equipment at least;

    a) four(4) months for first time trial hosts,    or    b) three (3) months for established trial hosts

    prior to the proposed trial date.

    If an inspection is done, the "trial site form" and "equipment approval forms"  must be included with this trial application.
