$25.00 Spay/Neuter Coupon Request Form

Please complete this form in order to request a mailed $25.00 Spay/Neuter Coupon from the Capital Area Animal Welfare Society. Please note the coupon is for $25.00 OFF the full price of a spay or neuter surgery. You will receive a confirmation email within 4 business days after the form is submitted. Once you receive your confirmation email, please send a Self Addressed Stamped Envelope with the printed copy of the confirmation email to CAAWS PO Box 77765, Baton Rouge, LA 70879. No request will be accepted without a copy of the confirmation email and a self addressed stamped envelope. Requests will only be accepted from residents of East Baton Rouge Parish and the surrounding parishes. (West Baton Rouge, Ascension, Livingston, East and West Feliciana, and Iberville Parishes.) There is a 2 coupon limit per person per month and a 6 coupon limit per person per year. Only one species is allowed per form.

Please Be Aware: It may take up to four weeks upon receipt of SASE and copy of form to recieve the coupon so please plan accordingly. If you need it more immediately please come to our shelter during our adoption hours, Saturdays between 11am and 3pm.
