$25.00 Spay/Neuter Coupon Request Form

Please complete this form in order to request a mailed $25.00 Spay/Neuter Coupon from the Capital Area Animal Welfare Society. Please note the coupon is for $25.00 OFF the full price of a spay or neuter surgery. Requests will only be accepted from residents of East Baton Rouge Parish and the surrounding parishes. (West Baton Rouge, Ascension, Livingston, East and West Feliciana, and Iberville Parishes.) There is a 2 coupon limit per person per month and a 6 coupon limit per person per year. Only one species is allowed per form.

Please Be Aware: It may take up to four weeks upon completion of the form to recieve the coupon so please plan accordingly. If you need it sonner immediately please come to our shelter during our adoption hours, Saturdays between 12pm and 4pm.
