    SONGWRITER'S NAME : The definition of any songwriter is a person who has either composed the melody and / or written the lyrics (or contributed in a major part to either of these aspects).

    CAE / IPI No. : If you are a member of a Performing Rights Organisation (such as PRS, BMI, ASCAP etc) please include your CAE / IPI (Membership) Number.

    % SPLIT : Co-composers normally have an equal split share of royalties.

    CO-COMPOSITIONS : Complete separate forms for different combinations of songwriters ensuring that all of the co-composers are correctly listed.

    AUTHORISED REMIXES : Please add the name/s and contact details for all original composers.
