As the adopter of the above named pet, I pledge to care and provide a warm and loving environment for my pet. 

I understand that pet ownership comes with responsibilities, both financial and emotional, which I will provide. I pledge to make arrangements to care for my pet when I travel and provide a safe environment, free from harm, neglect, and abuse.

I will demonstrate responsible pet ownership by providing all veterinary care including but not limited to health checkups, flea control measures, vaccinations, and training (if needed) to keep my pet healthy, obedient and disease free.

I understand I must comply with all state and local ordinances concerning pet licensing and vaccinations.

I also understand that rescued shelter pets may have been exposed to communicable diseases while at the shelter, and therefore, will not hold Love on a Leash Rescue, Inc. responsible for such diseases or incurred medical costs related to such diseases. I also understand that signs and symptoms of communicable diseases may not be present at time of adoption. I understand there is no “cooling off” period and assume all liability and responsibility for my pet as of the date of this contact.

If for any reason, I no longer want my pet, I agree to notify Leashes of Love Rescue, Inc. in writing, clearly stating the reasons I am relinquishing my rights and responsibilities of pet ownership. I agree to give Leashes of Love Rescue Inc. at least 72 hours notice to make arrangements for transfer and transport of my pet. I acknowledge that I will be responsible for all transport costs incurred to transport my pet back to where Leashes of Love Rescue Inc. deems appropriate as well as boarding costs (if any), if less than 72 hours notice is provided.

I understand that the adoption fee paid to Leashes of Love Rescue Inc. is considered a donation and will not be refunded for any reason.

All of the above information I have given is true & complete to the best of my knowledge. I agree to follow the Rules and Policies of Leashes of Love Rescue, Inc. I understand that it is my decision whether or not to adopt any particular dog. I will not hold Leashes of Love Rescue, Inc. responsible for any damage, injury, or harm caused directly or indirectly to any person or property by any dog I may decide to adopt.
