Affiliation Form

TBAWA offers Public Liability, Professional Indemnity, and Personal Accident insurance for players and volunteers (including club administrators) to its member clubs. The insurance is provided by Gallagher Insurance Group through Baseball Australia. It is the member club's responsibility to ascertain that the insurance cover provided is appropriate to its needs.

Coaches, Managers, and Umpires are included automatically in the teams fee.

Clubs must insure ALL tee-ball players are reported on this form along with all other volunteers per club (e.g., elected office bearers, canteen staff, ground helpers).

Member Associations whose member clubs do not affiliate with TBAWA must insure enough volunteers to cover also the office bearers and helpers from all their member clubs.

In addition, your affiliation includes cover for Association Liability Insurance.

Property Insurance with a base cover of $10,000 is provided as a baseline and included below for all clubs that are not a member of Baseball WA (Note: Membership with Baseball WA already includes that cover). A higher cover can be arranged on a per club basis upon written request.

Please note, this form must be filled out and signed by an appropriate member of the club's executive committee (or equivalent).

Any innacuracies in reported numbers may have a negative affect on the club including denial of insurance claims, denial of insurance, or removal of affiliation.

By signing and submitting this form, you confirm that these numbers are accurate and take personal responsibility for ensuring any changes to club registration numbers will be communicated to the TBAWA Registrar by emailing registrar@tbawa.org.au
