Mentorship plan

This form is to help you to plan and talk about your mentorship journey under the ASDP Mentorship Scheme. If you feel like you need to review your plan along the way, that’s perfectly fine, this plan is simply a tool for you to use and for ASDP to keep records. Your mentorship journey is unique to you so feel free to shape it as you wish, some mentorships keep going after the initial period and some have served their purpose after only one session. There are no right or wrong answers here, the key is that you’re both happy and in agreement with what you’ve decided.

Maximum length of the mentoring period under the ASDP Mentorship Scheme is 1 year and you’ll be sent an evaluation form to fill at the end of your journey.
