Rev Scott DeMarco - Wedding Intake

  • Thanks for taking the time to share this information! This will give me a lot of what I will need to plan your ceremony. Please fill out as much as possible and if things change we can always modify it.

The first and second entries for "Bride/Groom" just indicate to whom the information following that section applies.

    Thanks,

    Scott

If you are using a mobile device, I recommend that you rotate to "landscape mode" to access all fields.

  • The following is basic contact information for Scott's files.

  • This information is used for the writing of your ceremony. Start with yourself first, and then add your fiance's information in the next section, or vice versa.

  • If your name is difficult to pronounce, or has a unique pronunciation, how would you sound it out?

    For example: Yvette Huber = Eee-VETT Hugh-BARE

  • And now your fiance...

  • If your name is difficult to pronounce, or has a unique pronunciation, how would you sound it out?

    For example: Thomas Czerak = Toe-MAHS ZAIR-ack

  • If you do not plan on a rehearsal, please re-enter your wedding date and time. If you don't know the date and time please give me your best guess.

  • Please fill out the following family members only if they will be present at your wedding ceremony. For example, if your Grandmother is attending but will not be seated by an usher during the processional, please list them.

This does not include family members or siblings that are in your wedding party. You will enter them with your wedding party members later in this form.

NOTE: Please list all parents, step-parents, or grandparents if they will be attending (even if not necessarily participating in) your ceremony.

  • Wedding "Bridal" Party

  • Please enter the names of your wedding (bridal) party. If you know your pairings, please indicate with whom they will walk in the processional/recessional.

Note: After each name you will be provided with another entry. Keep adding your names until your wedding party is complete.

    Example:

Mary Jones                     Bridesmaid                     Tim Smith                        Groomsman

  • We will spend time planning your ceremony, so to void any conflicting information, please list your DJ, venue rep, or planner if they will be present at the rehearsal.

DJ's and venue reps often ask that you submit a processional order, for example, or may pressure you into a boilerplate version. This has become a "value-add" for DJ's and venues, and unfortunately a lot of officiants are not prepared (or don't care) to know otherwise. One of the reasons you hired me is that we will customize everything from start to finish--a good thing!

By supplying these contacts, I will make sure everyone is on the same page.

When it comes to the Ceremony (meaning Processional-->Ceremony-->Recessional, I've got it. I will run rehearsal from start to finish and gladly include venue reps, DJ's, etc in the plans we have made.

PLEASE LET YOUR DJ'S, VENUE REPS AND PLANNERS TO CHECK IN WITH ME ON YOUR CEREMONY PROCESSIONAL AND CUES.

  • Thanks for providing this basic information. I understand some of it may change, and we can make any adjustments when we meet. I look forward to seeing you!

    Rev. Scott DeMarco
