GRACE CHURCH RELEASE
Liability Release & Consent to Travel
I certify that I/my student am cognizant of the inherent dangers associated with participating in this activity and with the fact that participating in this activity may take place outside of, or off, premises. I hereby release Grace Church of Humble, its trustees, representatives, instructors, and agents from any liability whatsoever arising out of injury, sickness, or damage which may be sustained by me during the course of the Activity. I further agree to save and hold harmless Grace Church of Humble, its trustees, representatives, instructors, and agents from any claim by me, my family, estate, heirs or assigns arising out of enrollment and participation in the Activity.
This document covers all events/activities for the year of 2016.
Medical Release
I do further give my consent for the director or properly appointed staff member of Grace Church of Humble to secure the administration of medical treatment or medication for my child in case of emergency, and I do further agree to the performance of such treatment, anesthetics, and operations as in the opinion of the attending physician is deemed necessary for myself.
Disciplinary Agreement
I understand that, while I participate in these activities, it is my obligation to abide by the rules set forth by the sponsoring organization, its leaders and supervisory personnel. Any infraction of rules and/or conduct by me can result in dismissal from the program. In the event I am dismissed from the program, I, undersigned, agree to assume the costs of returning home. I also agree to forfeit any possible refund. (I understand that such action would only be taken under extreme circumstances.)
I further state that I am of lawful age and legally competent to sign this affirmation and release: that I understand the terms herein are contractual and not a mere recital: and that I have signed this document of my own free act and volition.
Rules
- All students will refrain from the use of profanity.
- Firearms, knives or weapons of any nature are prohibited.
- Alcohol, drugs or any form of tobacco are prohibited.
- mp3 players, video games, radios, etc. are not permitted during services.
- Dress in a modest manner, which exhibits Godly character.
- Physical or verbal altercations will not be permitted.
- No student may be in the dorm of the opposite sex AT ANY TIME.
- All students must be in their dorms at the time announced by the pastors or leaders with NO exceptions (unless special permission has been granted by dorm leader or pastors).
- Any students found outside their dorm past curfew will face disciplinary action based on the discretion of the pastors.
- Students may not leave the campground without Pastor Ty’s prior knowledge and permission. If any student must leave the campgrounds for any reason, only Pastor Ty or his designated assistant may hand them over to their parent or guardian. The guardian must be listed on this form and must present photo identifcation.
- Students may skip eating breakfast; however, no student will be permitted to skip eating lunch or dinner.
*Any banned items will be confiscated, and may be cause to be sent home for the week and proper authorities notified.
