    Credit cards are accepted for deposits only.   • ENROLLMENT AGREEMENT

    Please review the Enrollment Agreement to complete your application. Your signature affirms that you have read and agree to the terms and conditions below. 

  • Itinerary: I understand that it is Global Works’ intention to offer a similar itinerary as that
    described in the brochure. I also understand that locations, projects, and activities may be
    subject to change, and that itineraries may change. In all cases, Global Works will make a
    commercially reasonable effort to substitute an activity or location with something
    comparable.
    Tuition, Refunds, and Trip Cancellation:
    SSA Deposits: Summer Service Adventure Deposits are $800 and are fully refundable for seven days. After seven days, $600 of your deposit is refundable up to March 1st. After March 1st, the deposit is nonrefundable. Cancellations must be submitted in writing; the date such notification is received by Global Works is the cancellation date. You can purchase the Program Cancellation and Interruption Protection Plan (PIP) for 3% of your tuition cost and receive a tuition refund, less your $800 deposit. Full tuition and Program Cancellation and Interruption fees must be paid by April 1st. Please note that airfare is not considered part of the tuition, cannot be refunded, and is not covered by the Program Cancellation and Interruption Protection Plan.
    CGT Deposits: Custom Group Travel Deposits are $500 and are fully refundable in cases when the trip does not meet the minimum participant requirements. Should cancellation occur prior to 90 days before departure, the following specified costs are non-refundable: 46-89 days —50% of tuition price; less than 45 days from departure will receive no refund. Cancellations must be submitted in writing; the date such notification is received by Global Works is the cancellation date. You can purchase a Program Cancellation and Interruption Plan and receive refunds equal to 100% of the tuition less deposit under the policy's specified conditions. Full tuition and Program Cancellation and Interruption fees must be paid by tuition due date as financial protection begins at the time your first tuition payment is received and ends on the last day of the program. Please note that airfare is not considered part of the tuition, cannot be refunded, and is not covered by the Program Cancellation and Interruption Plan.
    Tuition includes: lodging, food, all transportation (not including airfare), all excursions, any activities and entrance fees specified on scheduled itinerary, full-time services of professional Global Works leaders, and all non-optional gratuities. The tuition does not include: passport and visa fees (non U.S. citizens must secure any necessary visas), airfare, baggage fees, airport departure taxes, excursions and activities not included on the itinerary, meals in airports or on travel days beginning or ending a trip, medical and evacuation insurance, or program interruption protection. Global Works’ refund policy is based on the fact that most program costs are determined on a per-group, instead of a per-person, basis. In addition, last minute cancellations often prevent other participants on the waiting list from participating. I understand that Global Works is not required to provide, and I am not entitled to, a tuition refund for late arrival, dismissal with cause, cancellation or early withdrawal for personal reasons, failure to acquire passports/visas/immunizations/airfare prior to a trip, or a medical emergency or injury during a trip. Any refunds will be made at the sole discretion of Global Works. Global Works strongly recommends purchase of Program Interruption and Cancellation Protection Plan (PIP), a tuition protection plan for medical conditions or family emergencies only. I understand that paying for the Program Interruption and Cancellation Plan, as stated on my invoice, indicates that I have read and agree to be legally bound to the terms and conditions stated in the agreement. Information about the Program Interruption and Cancellation Protection Plan has been made available to me. I understandthat PIP payment is due with the first tuition payment in order to be eligible for this coverage. I understand that Global Works does not represent or act as an agent for, and cannot control the acts or omissions of, any airline or transportation carrier, and hereby release Global Works and any of its agents from any injury, loss, damage, accident, delay, or expense arising from such acts or omissions. Additional trip cancellation insurance may be purchased through an outside trip cancellation insurance carrier. I understand that Global Works reserves the right to cancel a program that does not enroll to a minimum level and/or in cases where travel to a destination has been deemed unsafe. In such an occurrence, Global Works will make a commercially reasonable effort to offer a similar alternative program when available. If such comparable program is offered and refused by the enrolled, any paid tuition (not including the deposit) will be refunded. Global Works is not responsible for other costs, airfare, and incidental expenses incurred by a participant preparing for a cancelled trip.
    Passports and Visas: I understand that it is each participant’s responsibility to obtain a valid passport and visa (if appropriate). We suggest that this process be completed well in advance of departure. Passports and visas (if appropriate) must be valid up to six (6) months following the trip’s departure date. Non-U.S. citizens must contact the appropriate embassies and consulates to inquire about any visa requirements for all countries to be visited. Global Works is not required to provide any tuition refund for a participant's failure to acquire passports, visas, immunizations, or airfare prior to a trip.
    Rules and Regulations: I understand that for the health, safety, and welfare of its staff and
    participants, Global Works requires that the rules and regulations it establishes be observed. I agree that should the participant’s conduct, at the sole discretion of Global Works, be deemed to be in violation of Global Works’ rules or otherwise detrimental to the maintenance of standards or to the successful operation it’s programs, Global Works may dismiss him/her from the program. I agree that Global Works responsibility for the participant ends with his/her dismissal or withdrawal from the program. I understand that in cases of early dismissal or withdrawal, accompaniment by a Global Works leader on the flight(s) home will not be possible, however at the request of a parent or guardian, Global Works will attempt to arrange for the participant to be treated as an unaccompanied minor by the airline when possible. I agree that should my child/ward be dismissed from a Global Work’s program, I will make arrangements for, bear the cost of, and provide supervision and/or responsibility for my dismissed child/ward, even in situations that require extended international travel, and that no refund of the program tuition will be given for a dismissed participant. The following cases may be cause for early dismissal from the program:
    • Unsafe behavior, which includes, but is not limited to: failure to wear seat belts, life jackets, and other safety equipment, self mutilation, and/or body piercing.
    • Behavior that is destructive, abusive, violent, injurious to others or involves harassment, theft, vandalism, or chronic defiance of rules and regulations.
    • Exclusive relationships, sexual activity, unauthorized absence from the group, or failure to bide by curfews and/or other set expectations for behavior.
    • Possession or use of any weapons, non-prescribed drugs, tobacco products, narcotics, or alcoholic beverages.
    Personal Property Loss: I agree that Global Works will not be held responsible for the loss, theft, or damage of personal property or equipment. Petty theft is worldwide! Global Works strongly advises participants not to bring expensive cameras, electronic equipment, jewelry, sunglasses, etc. Personal insurance should be purchased to protect any valuable personal property.
    Health Forms: Global Works’ Health History Form must be completely filled out and signed by a parent or guardian (if participant is under 18 years) or the participant. The Physician Report and Examination must be completely filled out and signed by a physician who has examined the participant within one year prior to the start of the program. Acceptance into a Global Works program is contingent upon Global Works’ approval of the foregoing health forms. Failure to answer all questions accurately or the misrepresentation of the participant’s current or past physical and/or emotional condition may necessitate the dismissal of the participant, even after the program has commenced, at the participant or parent/guardians’ expense. Misrepresentation, failure to fully disclose information or failure to update Global Works with new behavioral developments or other pertinent health form information prior to the program may result in dismissal or rescinding of acceptance (even before a program begins) without refund.
    Medical Treatment: I understand that all participants must be covered by medical and emergency evacuation coverage in the destinations they travel with Global Works. In case of illness or injury, I grant permission to Global Works and/or its assigns, to hospitalize, treat and order injections, anesthesia or surgery for me or my participant son or daughter as may be appropriate. I further authorize Global Works to administer non-prescription medicine in the event of minor injuries and/or ailments to me or my participant son or daughter as may be appropriate. I understand and agree to pay all costs associated with medical care and transportation, including the cost of any evacuation, treatment, or medications. I understand that paying for the AXIS Emergency Medical and Evacuation Insurance, as stated on my invoice, indicates that I have read and agree to be legally bound to the terms and conditions stated in the agreement. The AXIS Emergency Medical and Evacuation Insurance agreement has been made available to me. I understand that AXIS payment is due with the first tuition payment in order to be eligible for this coverage.
    Photo/ Statement Release: I give permission to Global Works to use any photographs, videos, and writing of the participant and/or parent in Global Works promotional and/or educational publications and media. I agree to let Global Works list participants and parents, unless otherwise stated in writing, as a “Reference” in future Global Works promotional materials.
    Assumption of Risk, Release, and Indemnification Agreement: I understand and acknowledge that the Global Works program to which I and/or my participant son or daughter is applying, as may be appropriate, involves inherent risks that are beyond the control of Global Works. These risks include, but are not limited to: strikes, civil unrest, war, terrorist activity, weather, medical outbreaks, other possibly dangerous conditions beyond the control of Global Works, illness associated with an unfamiliar diet, risks of personal injury in motorized vehicles, participation in unauthorized and unsupervised activities while on the program, and the risk of personal injury involved in adventure activities or otherwise. I (participant) on behalf of myself, and I (parent/guardian) on behalf of myself and my participant son or daughter, accept and fully assume these risks of travel. I (participant) on behalf of myself, and I (parent/guardian) on behalf of myself and my participant son or daughter, agree to release, waive, covenant not to sue, indemnify and hold harmless Global Works and its agents from and against any and all claims, liabilities, demands, damages, costs, attorneys’ fees, losses, expenses, rights and causes of action of any kind or nature arising out of any loss or damage to property or personal injury, including death or illness, incurred by me or my participant child while participating on a Global Works program. I further understand that Global Works subcontracts various transportation companies, travel agencies, and adventure outfitters, and with this understanding, I (participant) on behalf of myself, and I (parent/guardian) on behalf of myself and my participant son or daughter, agree to release, waive, covenant not to sue, indemnify and hold harmless Global Works and its agents from and against any and all claims, liabilities, demands, damages, losses, expenses, rights and causes of action of any kind or nature arising out of or caused by any act, error, omission, or default by any such subcontractor. Should it become necessary for Global Works to advance monies on behalf of a participant for any reason, I (participant) on behalf of myself, and I (parent/guardian) on behalf of myself and my participant son or daughter, agree to reimburse and indemnify Global Works for all such expenses. Any individual upon bringing legal action against Global Works will be responsible for all legal fees and costs, including reasonable attorneys’ fees, incurred by Global Works in the defense of such action. If one parent or guardian of a participant does not sign this Enrollment Agreement, the signing parent or guardian indemnifies Global Works and its agents from all claims by the non-signing parent or guardian. If any provisions of this Enrollment Agreement are held to be invalid, void or unenforceable, all other provisions of the Enrollment Agreement will nevertheless remain in full force and effect. Any and all actions arising out of or to enforce this Enrollment Agreement shall be heard in the exclusive jurisdiction of the courts located in Boulder County, Colorado, United States. The construction, interpretation, performance and enforcement of this Enrollment Agreement shall be governed by the substantive law of the State of Colorado, U.S.A., notwithstanding its choice of law rules. This Enrollment Agreement constitutes the entire agreement between the parties regarding the subject matter contained herein. This Enrollment Agreement supersedes all prior agreements, proposals, understandings, statements and/or representations, whether written or oral, concerning the subject matter contained herein. This Enrollment Agreement may not be altered, amended or extinguished except by a writing that expressly refers to this Enrollment Agreement and that is signed by all parties subsequent to the execution of this Enrollment Agreement. I believe that I or my participant son or daughter is capable of handling the physical and emotional experiences entailed in a Global Works program.
