  • Please check the attached information upon receipt and verify all information is correct. Tailored Destination will not be responsible for omissions or errors if not brought to my attention immediately. Payment must be received by the due date to avoid vendor cancellation.

    I understand all components of my itinerary.  This includes dates, times, airline, destination, cancellation/change penalties, baggage fees, and documentation requirements for traveling internationally.  All travelers’ names and birthdates are legal and are spelt the exact same as the government issued photo ID (license or passport) being used at the airport.  I have read the attached documentation and responsibility information.  I have reviewed my travel documents for accuracy upon receipt and understand that changing any aspect of my travel arrangements may result in the payment of additional money. 

  • Documentation

    U.S. citizens travelling to any destination outside of the United State will be required to present a valid U.S. passport.  Passports must be valid at least 6 months prior to your return date. Some countries require a visa for transit or entry.  Passengers are responsible to ensure that they have all proper documentation for entry.  All names on documents must match the legal name on you photo ID and travel document information must match tickets.  Immunizations may be required.  Please check http://travel.state.gov for further information.  Failure to comply with these regulations may result in denied boarding, denied entry and or government fines. Full passports are highly suggested for cruise vacations. 

     

  • Travel Insurance- Waiver

    I understand that I may purchase travel insurance to cover certain risks inherent in travel such as supplier bankruptcy, cancellation fees assessed by suppliers and the inability to travel due to a medical or personal emergency. Please check and initial your acceptance or refusal of travel insurance below. Understand that cancellation fees may be imposed on any or all parts of travel. I have been advised of all supplier’s refund charge and cancellation fees and penalties.  I am aware that the airline ticket may be subject to the loss of some (or all) of the fare, should I elect to change or cancel this reservation. Therefore,  Tailored Destination strongly advises that everyone protects their investment by purchasing some type of travel protection program. 

    NOTE: to cover pre-existing conditions insurance must be purchased within 14 days of trip deposit

    I assume and understand that there is a risk involved with my travel and my travel activities and/or excursions.  I acknowledge that I was informed about my options but I declined the insurance.

  • I have been advised that cancellation, change or refund penalties do apply to the travel associated with these documents and I fully understand the terms and conditions of such penalties.

  • Responsibility

    Tailored Destination offers retail travel services to customers, which are provided by separate and independent vendors or travel services. The customer agrees and understands that Tailored Destination is not responsible for any operating failures, loss, damage, delay or injury to you or members of your traveling party, nor for consequential damages, which may result from failure, delays or injuries, which arise our or  during your scheduled travel.  Customer agrees and understands that Tailored Destination is not responsible for any damages direct or consequential as a result of Acts of God, social or labor unrest, political or national strike, terrorist activities, mechanical failures, climatic conditions, or any other actions or omissions beyond Tailored Destination's control.  In the event an airline or travel vendor declares bankruptcy it is not obligated to carry you or to provide funds. Money given to my agency immediately becomes the property of the airline or travel vendor as required by law. Tailored Destination is not allowed to provide refunds for vendors who have declared bankruptcy.  If an airline or travel vendor declares bankruptcy, it might continue services or stop completely   other airlines or travel vendors might provide alternative services under limited circumstances.
