  • This production process involves the following activities:

    Playing theatre games, eating/drinking snacks/meals, performing theatrical scenes, acting, dance, music, rehearsal, building sets, props, and costumes.

    The organizer has identified that the activities will involve the following risks:

    cuts,strains,sprains,dislocations,dehydration,infections,emotional distress,fractures,abrasions,injury from a fall,exhaustion,concussion,joint or back injuries,insect bites,muscular skeletal injuries,injury to the head, face, or body,injury from contact with other participants,risks involved with physical activity,injury or illness related to heat or cold weather exposure

    Participants are expected to adhere to the following General Rules and Guidelines:

    Listen to and stay with chaperone, coach, or rehearsal supervisor, stay hydrated, be respectful, stay with the group at all times, Be respectful of rehearsal and performance spaces, exercise situational awareness, safety is always our top priority.

  • Assumption of Risk, Release of Claims, Hold Harmless Agreement, Indemnification, and Necessary Medical Information

  • In consideration of my child being allowed to participate in this production of The Tempest hosted by 525 Arts Collective, I, the undersigned parent or guardian, agree on behalf of myself and my child as follows:

    I have read, understand, and do accept the General Rules and Guidelines for this production as outlined in the Program Activities, Risks and Rules section above. I and my child are bound by the General Rules and Guidelines and/or any additional safety or conduct directive given by staff to participants. I agree that my child must conduct him/her/themself in a respectful manner so as not to disrupt or interfere with others’ participation. I understand a failure by my child to comply with the guidelines and/or if my child conducts him/her/themself in a disruptive manner, my child may be removed from the event, with no refund provided.


    I grant my permission for my child to participate in all Program activities during the event, including but not limited to the activities outlined in the Program Activities, Risks and Rules section above. 

    I WAIVE, RELEASE, AND DISCHARGE for my child and myself, my child’s and my heirs, executors, administrators, legal representatives, assigns, and successors in interest (“successors”), 525 Arts Collective and all their agents, officers, and employees (all collectively referred to as “releasees”), from all claims, demands, and causes of action of any kind, including claims for negligence, invasion of privacy and/or any other claim, which may arise from or be related to my child’s participation in and/or appearance at the event, including but not limited travel to and from the event.


    I and my child fully realize the risks associated with participation in the event, and I and my child fully ASSUME THOSE RISKS, including by way of example, but not limited to: the possibility of serious physical and/or mental trauma or injury (minimal, serious, catastrophic, death), uneven terrain, injury from extensive walking, and other harmful interactions with other participants, releases, or other third parties. See the Program Activities, Risks and Rules section for more details.

    I agree that if my child requires emergency medical treatment as a result of any accident  or illness arising during participation in the production of The Tempest, I expressly consent to such treatment. I further acknowledge that 525 Arts Collective  will not provide health and accident insurance for my child, and I agree to be financially responsible for any medical bills incurred as a result of emergency medical treatment.


    Unless I opt out on behalf of my child by contacting the Founders at least three days prior to the event, I waive any rights to and consent to the recording and use of my child’s image and likeness by releasees. I understand and voluntarily authorize the releasees to: (1) record my child’s participation and appearance on any medium and in any manner (collectively referred to a “Photographs”); (2) use and/or publish my child’s name, image, likeness, voice (collectively, “Likeness”) in connection with or separate from the Photographs; (3) exhibit and distribute—in hard copy or electronically—such Photographs and/or Likeness in whole or in part for any communications, educational, marketing, advertising, publicizing, promotional, and/or any other marketing-related purpose. I waive any right that I or my child may have to inspect and/or approve any finished Photographs or Likeness products or the use to which it may be applied, and I understand and consent that neither I nor my child will receive financial compensation in exchange for use of the Photographs and/or Likeness. 

    This document is a continuing consent, waiver, release, and assumption of risk with no limitations or reservations, unless and except those stated herein. Any copy of this document has the full force and effect and is binding as the original.
