As a participant in this program, I hearby waive all claims and/or course of action including negligence against Lady D's Sewing Studio, Demeta Peterbark and for any accident that may occur to myself or my child while participating in the sewing classes and all sewing activities held by the instructor(s). I have carefully read this agreement and fully understand it's contents. I am aware that this is a Waiver, Release of Liability and Assumption of Risk agreement between me and Lady D's Sewing Studio, Trustees, All Officers, Directors, Employees, and Agents arising out of or in any way connected with my participation in the above designed activity.
