You have chosen to have a technician come to your location and fix your electronic device onsite. Therefore you give University Technicians' service technicians the full authorization fix your electronic device onsite at the location you've indicated within this form. All service is prepaid in cash or card upon arrival. Parts will be ordered additionally

TERMS AND CONDITIONS:

All repairs are completed based upon service requested. If any new problems are found during the service, you will be notified immediately through the contact information provided. If any damages are made while servicing it will be replaced at no charge. In the case that no proof is given by the owner that damage was indeed made by a technician of University Technicians, we will not be held responsible. There are no refunds for service completed. Refunds will only be given for parts that were ordered and purchased. Upon completing this form, you agree to all the terms and conditions above, and this will serve as a consensual electronic signature. You submit that all of the information provided is true. Any fraudulent or stolen devices will not be serviced.
