If the model has earned an excess of $2,500 in prior performances, has in place a Child Performance Trust Account or a 529 Qualified Tuition Program and the model is subsequently selected as a model, then the authorized parent or guardian must supply information necessary to enable Perform Group to deposit the funds required by Pennsylvania's Child Labor Law, 43 P.S. § 40.5 into the account established for the model. The authorized parent or guardian understands that if such account is not set up for the benefit of the model and Perform Group is not notified of the existence of the account within 15 days after commencement of the services, then Perform Group will be required to transfer the appropriate funds to the State Treasurer for the placement into an account set up for the benefit of the model as required by 43 P.S. § 40.5.
