Please note the following:
- Due to the large number of submissions, we can only notify individuals that are selected to appear in our catalogs.
- If selected to appear, we require that you only model costumes for Curtain Call^®. This exclusivity applies only to other dance costume and dancewear catalogs during the current catalog season.
- Photo shoots take place during the week from January through May in Central PA. Models must be available to travel to Central PA at their own expense. We do compensate models for their time, but do not reimburse for travel expenses.

If yes, does the model currently have a Child Performance Trust Account or 529 Qualified Tuition Program in place?
If the model has earned an excess of $2,500 in prior performances, has in place a Child Performance Trust Account or a 529 Qualified Tuition Program and the model is subsequently selected as a model, then the authorized parent or guardian must supply information necessary to enable Perform Group to deposit the funds required by Pennsylvania's Child Labor Law, 43 P.S. § 40.5 into the account established for the model. The authorized parent or guardian understands that if such account is not set up for the benefit of the model and Perform Group is not notified of the existence of the account within 15 days after commencement of the services, then Perform Group will be required to transfer the appropriate funds to the State Treasurer for the placement into an account set up for the benefit of the model as required by 43 P.S. § 40.5.
Measurements:
A leotard must be worn when taking measurements, not street clothes. Have measurements taken by the student's dance teacher if possible, making sure they leave no room for growth.
