Adult Achievement Award Endorsement Form
Endorsements can not be completed by the nominator and must be completed by someone else who is familiar with the candidate's service to Girl Scouts.

Refer to the appropriate award description on our website and include how the candidate meets the requirements for the award. Answer the questions as if the person reading the nomination does not know the individual.

All Adult Achievement Award nominations and required endorsements are due by February 1st.
