  • To be completed for each child (Beginners through 8th grade) during the first week of attendance at The Honor Roll School. This includes: Student Information, emergency contacts and permission to arrange for emergency medical attention, Media Release, Authorized Pick-up Information, Upload of latest Immunization Records.

    If your child is a returning student to THRS, you may want to edit last year's Student Information Forms (no need to fill them out from scratch). If you are editing an old submission, you must revise and change the grade level, t-shirt size, etc. Search in your email for the subject "We have received your response for Student Information Forms". Within this email you will see a link for "Edit Submission". Using this link, you will be able to retrieve last year's forms, make the necessary changes (including grade level, siblings grade levels, pick up authorizations, dates of signatures, etc). Don't forget to submit the forms once you are done!

  • Throughout the school year, students may be highlighted in efforts to promote The Honor Roll School's activities and achievements. Students may appear on pictures, video, printed and/or electronic/digital media, including our morning broadcast. This release authorizes The Honor Roll School to use and publish the picture, likeness and/or testimonial of your child in such materials. This is a general release between the parent/guardian and Spring Education Group.

  • IT IS HEREBY AGREED AS FOLLOWS: That, I, the undersigned, for good and valuable consideration, the receipt of which is hereby acknowledged, hereby grant to Spring Education Group and its parent corporation, subsidiaries, affiliates and other related companies (collectively, "Spring Ed"), and all of its or their respective officers, directors, agents, employees, partners, licensees, shareholders, predecessors, successors and assigns, solely for Spring Ed promotional purposes (the "Permitted Use"), the right to use and publish the picture, likeness and/or testimonial of my child.

    I acknowledge that the Permitted Use includes any medium now or hereafter known, without restriction as to manner, frequency or duration of usage, and shall be without compensation of any kind. I agree that my child's picture, portrait, likeness and/or testimonial may be used in conjunction with any visuals, copy or other elements Spring Ed may determine, subject to the terms of this general release. I further agree that all such materials produced hereunder are and will remain the sole and exclusive property of Spring Ed and will not have to be reviewed with me prior to their use, and that Spring Ed will have no liability to me resulting from the Permitted Use.

    I acknowledge that if in the future I submit a written withdrawal of the foregoing permission to the school's principal, such withdrawal of permission will pertain only to future or new materials, and will not terminate the Permitted Use with respect to any material previously produced or used.

  • Authorized Pick-up Information

  • Emergency Alerts Notifications (Text-Messaging System Notice)

  • The Honor Roll School uses Alma emergency messaging system to notify parents of certain emergency school closures. In the event of inclement weather or any emergency situation, THRS will text and call all parent mobile numbers as they appear in their Alma account.

    Due to strict fines imposed by cell carriers for sending text messages without the person's consent, the school cannot enroll any parent in the system. Therefore, PARENTS MUST OPT-IN THEMSELVES.

    To opt-in for emergency alerts, parents must log into their Alma account, go to Settings, and select either "Phone call" or "Text messages" option under Emergency Notifications. Make sure to click on "Save" after you have made your selection. 

    Parents must also check the phone number we have on file by going to their Alma account, clicking on their name at the top right corner, and clicking on the "Phone" section. From here, parents can check the box to indicate this number can be used to receive text messages.
