• Quality Assurance Audit Appeal

    • Prior to submitting an appeal Elmhurst encourages its Members to read the audit feedback, and if necessary, contact our Technical Support team to test understanding and resolve any initial queries.
    • Please ensure that you have familiarised yourself with Elmhurst’s  audit appeal process in our Service Level Agreement prior to completing the appeal form below
    • By submitting your appeal you agree to be bound by the terms of the appeal process
    • All mandatory fields must be completed in full as incomplete appeal forms cannot be accepted
    • There is no charge to Members where an appeal is successful. Revised audit feedback will normally be issued within 48 hours of Elmhurst’s decision and your QA record will be updated accordingly
    • Unsuccessful appeals will incur an administration fee of £25.00 +VAT which will be added to your next monthly invoice

    Please provide specific reference (including page/paragraph number) to the relevant sections of supporting documentation (e.g. Elmhurst's RdSAP manual, RdSAP Conventions, Technical Bulletins, Scheme Operating Requirements, evidence requirements etc).which support your appeal above. PLEASE NOTE – your appeal cannot be considered if this information is omitted.
