Fill this out and submit it at least 24 hours before any coaching or focus session with DHC.
On weeks when we're not meeting, please fill it out by Sunday and submit, so you stay top-of-mind.
It doesn't have to be perfect. In fact ... it has to be IMperfect.
If you find you start obsessing over one part of it, just say something like "I'm obsessing about this question because I'm worried that ..." [and fill in that blank] - then just do the best you can answering the question ... imperfectly.
I encourage you to write the answers in a text editor, and then cut-and-paste them into this form instead of writing right into the online form. These forms are pretty stable, but I've lost internet access in the middle of filling out an online form and it's not fun!
You'll get an email with all your answers after you hit 'submit' - and don't worry if you write something wrong. You'll be able to change any answers in our session - and I won't hold you to anything you write.
Even if these questions seem irrelevant to what you most want to get out of the session, please fill them out anyway - they'll help me keep oriented to where you are. And don't worry if you find yourself repeating things - that's fine.
