The following are required for renewal:
Notarized and completed application.
On application, complete inventory amount for retail package.
On application, complete seating capacity for retail consumption.
Notarized SAVE affidavit for owner and licensee.
Notarized Consent Form for background check and copy of government issued photo identification for licensee and any owners owning 5% or more.
Please submit renewal license application before November 15, 2016 to prevent delay in processing your application. If your application is submitted online, an invoice will be emailed to you within one (1) business day. Once you receive the invoice, please follow the provided instructions to submit payment. Your application will be processed after payment is received. Penalty and interest will be applied after December 31, 2016.
