    This completed form should be submitted at least 4 weeks prior to an event. Due to copyright considerations, this form should be completed before any music is planned and rehearsed.

  • "Live stream" means an event will be streamed live/at the time of the event through a web page.

    "On-Demand Archive" means an event will be saved to our Chapel archive website and made accessible for users to select and watch/listen to video or audio content when they choose to, rather than having to watch at a specific broadcast time.

    "Downloadable Online" means an event will be available for download directly to a device.

    Note: Each option above requires specific copyright permissions to be secured beforehand from the publisher/copyright holder. PSG is available to answer any questions you might have about each option, but the university only has licensing to allow downloadable content when the music performed is found within the CCLI catalog.

    Permissions obtained directly from a copyright holder should be provided to PSG in writing prior to the event and include expressed permission "for use in live performance, interactive and non-interactive video and audio streams, and online download." 

  • ** The reporting of every song and loop performed in an event is required by our licenses at CU.
    While researching, please ensure that the actual arrangement for each song being performed matches the listing in the Performance Rights Organization (PRO) catalog. Enter in the fields provided below, the title, author, and workID from the PRO catalog entry.
