NOTE: Participants are required to have a laptop computer for participation in the training. Laptops MUST have valid virus protection software installed prior to attendance.
You may make changes to your application multiple times before submitting, if you accidentally close the window or refresh the screen while you are working on the form, your entries will automatically save with the last updated info. Please note that this feature saves the data into the local browser storage of the user's computer or device, so it's required to continue using the same computer or device and browser if you choose to end your session and return later.
The application contains four sections, Applicant Info, Computer Literacy, Policy Proposal, and CV with Letters of Support. Additionally where ethical clearance or IRB approval is required for data access, a Data Clearence Letter will be required with your application submission. Instructions for filling out the application are provided below.
If submitting online, additional instructions and examples are provided in the appropriate fields in the application. Once you submit the form, all fields will clear and you will no longer be able to make any changes.
You may begin your application even if your data access or support letters are not ready, but applicants will not be able to submit their applications without uploading the additional requested materials (ex. CV, supervisor/sponsor support letters, data access letters).

Computer Literacy
Indicate your level of profiency and use of the following programs, Word, Excel, Epi Info, STATA, SAS, SPSS, and Internet Search Engines
Provide the frequency in which you use a computer for data analysis.
Policy Proposal
The problem statement can be written in a word processor, copied and pasted into the field, please do not include any table or figures. The problem statement should be written using 12-point Times New Roman or 11-point Arial as the minimum font size for the text of the proposal. The statement should include a detailed description of the problem or issue you are focused on. Include in your statement the health policy implications and/or impact.
List the primary audience and stakeholders who will be most interested in the policy implications and thus more likely to refer to the brief. Stakeholders should be included from various sectors (ex. government, political, commercial, NGO, civil societies, etc) at the national, regional, and local levels. Also consider stakeholders who would be in favor of the brief and those who would be against.
Identify the data sources that you plan to use for analysis to provide evidence-based support for your policy brief.
The following documents are required with the application. These may be uploaded prior to application submission.
PARTICIPANT CV are required to provide additional insight into the background, experience, skills of each participant. This will help to ensure that the minimal requirements for participation into the training program are met.
LETTER OF SUPPORT FROM SUPERVISOR/SPONSOR should acknowledge acceptance of the applicant the Data to Policy Training program and the willingness of the Institution to support his/her work during the training and mentorship phase of the program. Sponsors are responsible for taking charge in guiding and supporting the applicant during the proposal development process on a regular basis. Supervisors are responsible for ensuring that applicants are supported within their institutions to successfully achieve milestones in addition to their day-to-day tasks. The letter of support should indicate the role expected of the applicant in the broad framework of the sponsor/supervisor relationship. *All applications must contain a letters of support from– one supervisor and a sponsor. For participants whose sponsor and supervisor are the same person, only one letter of support will be required.
DATA ACCESS LETTER will be required for those participants who will need ethics or IRB approval to access data for secondary data analysis. A copy of an IRB or ethics approval or a determination of non-human subject research letter will need to be submitted with the application.
Once you have completed the application and uploaded the required documents, you may then submit.
THE APPLICANT CANNOT ACCESS HIS/HER FILES FOR EDITING OR ADDING MATERIALS AFTER SUBMISSION.
An error will occur for required fields that are incomplete will prevent the application submission. These require fields must be filled before you can submit. You will receive a confirmation when the application has been successfully submitted.
