LAKE ERIE LABRADOR RETRIEVER RESCUE, INC. A 501(c)(3) Organization Adoption Application

  • Are you planning any vacations in the next 2-3 months?

Yes: STOP. Please continue the application process when you will have 2-3 months time to help a dog get settled in the home.

No: Please continue the application process...

  • Another important reminder: if you are ready and available to adopt now, please continue with the following question. If you are not currently ready to adopt, please STOP and return to this form when you are ready.

  • Please read: Any dog moving to a new home takes time to settle into a new home and often will regress in some behaviors during the adjustment period. It is part of the bonding process.

We have short term fosters who work with our labs to help identify and begin working with the dogs to help them integrate into a home environment--they identify the behaviors the lab needs to work on (potty training, crate training, leash walking, staying alone in the home, counter surfing, building confidence, etc.) and begin working with them so that the potential adopter can identify whether the dog would be a potential fit.

The adopter will be expected to continue working with the dog. We also have longer term fosters where the foster works extensively with the dog and shapes the behavior so that while the adopter may need to give the dog a "refresher", the dog is comfortable with most behaviors in the home.

  • If yes, STOP. We do not adopt to homes with children under school age.

If no, please continue with your application...

  • If No, STOP. We do not adopt to first time dog owners who are renters.

If Yes, please continue with your application...

  • Please note, we will be asking you to contact your vet for written verification that you have kept your current/most recent dog up to date with annual vaccinations, annual heartworm testing and year-round use and purchase of heartworm preventive medications.  We require documentation showing this from either you or your vet.

  • If you have been with this vet for less than 3 years or you use more than one vet, please provide the contact information for that prior/additional/other vet.

  • Please provide two personal references (these may not be family members):

  • Note for above question:

Please contact your references to let them know that you have listed them - so they will be expecting a call from a # they are unfamiliar with and that you authorize them to disclose information to our rescue. We will not be able to proceed until we have spoken with both of them.

  • While every effort has been made to evaluate the dogs we either foster or to which we might refer persons seeking to adopt, we do not guarantee the temperament or physical soundness of any dog that may be placed through the rescue process.

We may require YOU to provide the documentation of veterinary care.

I/we further agree to release and hold harmless Lake Erie Labrador Retriever Rescue, Inc., its Trustees, officers, directors, volunteers, and foster families from any claims, damages, costs, or actions incurred as a result of any adoption through its auspices or caused by the actions of any dog adopted through its auspices.
