This form must be submitted by the player who is requesting to be transferred from one USTA team to another one OR by the staff of the facility of the team the player is requesting to be transferred to OR by the USTA LLC. Captains may NOT submit this form for a player.

A player who has played in a match or who has accepted a default cannot be transferred from one team to another one.
