- Photo must be a JPEG/JPG file, file saved as YOUR FIRST AND LAST NAME!
- Do not take a photo of a photo or provide a photo with a copyright logo.
- Photo must be in color and taken against a solid background.
- Photo must be professional in nature - NO duck lips, hats, sunglasses, or pics with your head to the side.
- We must see your whole face.
- Don't forget to smile!
After submitting your photo, your photo and information will be reviewed and verified. Auxiliary Services reserves the right to deny a form/request a new photo.
