Payroll Deduction: when an employee wants to take payment for their uniform out of their paycheck. Please have servers and bartenders ring in their uniforms as opposed to using this form. This form was created for hourly employees who do not receive tips.
Reissue Form: when an employee’s uniform is obviously worn out, to the point it is unacceptable to wear out on the floor, OR you have an employee needing a shirt due to a promotion. You can only reissue two shirts to an employee within a reasonable time frame (a year). Since we only issue employees two shirts upon hire, we will only replace those two. We will NOT replace any “extras” they’ve accumulated throughout their tenure.
