Event Submission Request

Please complete and submit one form per event no later than (14) business days prior to the event. Example: If your event takes place Saturday April 1st, the form must be filled out no later than Monday March 13th. The Fraternity holds the right to decline any event that depicts any of the following:
- Alcohol
- Explicit language
- Sexual Content
- Hazing
