Please fill out the sections that are applicable to your order - and be sure to enter in the CAPTCHA (the word in the box) before selecting "send" at the end of the form. An acknowledgement will pop up that your submission was sent and received.
Your submission will be reviewed and if additional information is needed, you will be contacted either via email or phone. An invoice will be generated based on the Order Form and sent to you to review.
Shipping will be calculated based on what has been ordered. If applicable, shipping and handling will be added to the overall order. If in Illinois, 10.25% sales tax will also be added. Note: Wrapping paper is shipped directly from the manufacturer and therefore that shipping rate remains as quoted.
Payment must be recieved before your order is placed. Payment options are via PayPal or Chase Quick Pay.
