INSTRUCTIONS:

Please complete this application form, making sure to provide all required information.

You need to start and complete this application all at one time. In other words, you cannot save a partially filled in application form and come back to it later, thus we recommend giving yourself ample time to complete the application (which includes writing two short essays).

Student Essays: This application includes two short essays (minumum of two pages each). We recommend that you write your essays in MS Word before you start filling in this application, then copy and paste your essays into the text boxes for those questions.

Application Fee: Payment of a $55 NON-REFUNDABLE APPLICATION FEE is required to submit this application form.

OFFICIAL TRANSCRIPT: Please send an official copy of your transcript to CCSP at 317 Antoni Glen #1306, Escondido, CA 92025.

THREE REFERENCES: CCSP needs three references, a Faculty Reference, a Student Affairs Reference, and a Personal Reference. Select your references then refer them to the following page on the website, https://www.creationcsp.org/how-to-apply, where they can complete their reference forms online in step 3.

OFFICIAL SCHOOL VERIFICATON: CCSP needs to receive official verification that you are permitted to attend the program. This requires the appropriate school official--usually your study abroad director and/or registrar--to sign and mail the following verification form to 317 Antoni Glen #1306, Escondido, CA 92025. Download the School Verification Form at this link.

Applications are considered under a policy of rolling admissions. To be considered for admission to CCSP we must first receive all of the above materials.
