By typing my full name, I agree and understand that: The Joy Committee Inc makes not implied or express warranty with regard to this animal. The animal is fostered "AS IS". It is understood by foster that should the animal cause personal injury or property damage after possession this is assumed by the individual pet foster, The Joy Committee Inc shall in no way be liable in whole or part for any damage. The individual hereby assumes all liability for the animal or any injury or damage that may be done by it while in your possession.

I understand that The Joy Committee Inc does its best to temperament test its animals, test for aggression, health issues, or any other problems, illnesses or problems, however there are times that unforeseen illnesses, aggressive or destructive behaviors may arise. I understand and hereby release The Joy Committee Inc from liability, I hereby release The Joy Committee Inc and its affiliates of any personal injury, or damage to any structure.

I certify that the above information is correct, and I understand that the information will be verified. I understand that by submitting this form electronically, I agree to release and covenant to hold harmless THE JOY COMMITTEE INC and it's members from any claims, damages, costs, or actions incurred because of the care or actions of the foster dog. I accept full responsibility for the dog(s) actions at all times while under my care, and release THE JOY COMMITTEE INC from any liabilities or damages that may be incurred because of fostering such dog(s). I agree to have THE JOY COMMITTEE INC complete reference call checks and conduct a home visit inspection to be able to approve my foster application. I agree that if I'm unable to foster the dog(s) anymore that I will return the dog(s) to THE JOY COMMITTEE INC and give THE JOY COMMITTEE INC a 4 week written notice to find another suitable foster for the dog(s). Futhermore I understand that returning a dog is very difficult on the rescue and I will work the the rescue fully to work with my foster dog(s). I understand that fostering is a full time job and can be difficult at times and I understand that this will require work on my behalf.
