The Holley Institute's Carls Family Village

 Everyone is expected to cooperate with the Holley Institute's Carls Family Village policies and procedures, which helps ensure the health and safety of all participants.

I understand and agree to: 

1. Treat staff and other participants with courtesy and respect.
   Communicate respectfully and not use inappropriate, derogatory, or vulgar language.
2. Wear appropriate clothing for all activities.
3. Social cruelty (bullying, teasing, put-downs, etc.) are not tolerated and may be grounds for immediate dismissal.
4. Obey all rules
5. Participants should follow directions from staff regarding electronic devices, including, but not limited to, cell phones, laptops, and iPods/iPads, iPhones, and other mobile devices. Electronic device use is prohibited during class time and group activities. Staff reserve the right to ask participants to put electronic devices away who are using them at inappropriate times.
6. Participants are expected to take part in all program activities to the best of their ability

I understand and agree to the above Code of Conduct. I understand that if I do not cooperate with the Holley Institute's Carls Family Village policies, I may be asked to vacate the premises/programs.
