I understand and agree to the following:
If awarded this scholarship, a total of $2,500 will be paid annually to my institution and can be renewed the second year per committee review for a total of $5,000. I must submit an official semester schedule of classes, maintain an annual GPA of 3.0 or above, stay in good standing with the institution and submit an annual essay on ongoing financial need before the beginning of the second year.
This scholarship will also provide mentoring throughout college from the board members to assist with any difficulties that may arise and to help me achieve graduation in my chosen field.
I must notify the scholarship board of my current mailing address, telephone number, and email address before the start of each semester. I must be willing and available to communicate with the members of the scholarship board. Failure to do so may result in the delay of my award disbursement.
Application is due May 11, 2018. Award is to be announced at the 2018 TRHS Sports Banquet. I must be present at this banquet in order to be awarded the scholarship.
If awarded, I will be presented with a welcome packet at the banquet. I must read and sign the included acknowledgement form and return via email within 30 days.
