FOR THIS TYPE OF EMPLOYMENT, STATE LAW REQUIRES A NATIONAL AND STATE CRIMINAL HISTORY BACKGROUND CHECK AND HAVE A LETTER, PROVIDED BY THE INDIVIDUAL, FROM THE CABINET FOR HEALTH AND FAMILY SERVICES STATING THE EMPLOYEE IS CLEAR TO HIRE BASED ON NO ADMINISTRATIVE FINDINGS OF CHILD ABUSE OR NEGLECT FOUND THROUGH A BACKGROUND CHECK OF CHILD ABUSE AND NEGLECT RECORDS MAINTAINED BY THE CABINET FOR HEALTH AND FAMILY SERVICES AS A CONDITION OF EMPLOYMENT.

I authorize the McLean County Board of Education to make any and all necessary and appropriate investigations to verify the information contained herein, including criminal records, educational credentials and work experience checks. References obtained are done so in confidence and I understand that my rights to review any reference material is waived.

I understand that the McLean County Board of Education is required by Kentucky law to obtain criminal history background information on applicants for employment.

I also understand that I will be required to enroll and participate in an appropriate state retirement plan immediately from point of employment in a regular status position - failure to do so shall result in termination of employment.

Prior to employment, I must provide information related to identity and employability. Failure to provide appropriate documentation for verification of employment eligibility shall result in immediate termination of employment and/or offer of employment.

I acknowledge that my employment may be terminated, and any offer of employment , if such is made, may be withdrawn, with or without cause and without prior notice at the option of the McLean County Board of Education or myself.

Candidates employed for a classified position must file a:

3. Federal and State employee's withholding exemption certificates

4. U.S. law requires that, if hired, you must furnish your social security card* or one of the following documents within 72 hours of starting work.

- Current INS forms with employemtn authorization stamp
- U.S. passport
- Voter's registration card
- U.S. military card or other draft card

A salary check cannot be issued until the items listed above have been completed and filed in the Superintendent's Office.
