Students that achieve an overll average of 95% or more for each module are eligible for the Academic Achievement Certificate. Please turn this form in at the end of each module with your grades for those students who are eligible. Students that receive a certificate all four modules of their program will receive honors at graduation. Certificates will be awarded the following week.
