Class/Program Refund/Credit Policy
Please read and check the box next to each section number. By checking each box you acknowledge that you have carefully read each of the policies below.

No refunds will be given once registered for a class or program without a physician's note. With a physician's note, a credit voucher for the unused portion of the class/program will be issued.
Refunds/credit vouchers will not be issued when payment for the class/program is less than $5.00.
Classes/Programs cancelled by a program director: Full refund/credit voucher.
Some programs require a deposit/registration fee that is non-refundable. Participant cancellation in these programs will result in the loss of the deposit/registration fee.
Credit vouchers will expire 12 months after date of issue.
Refund checks will be issued via mail within 2 weeks of submitting this form.
Refunds/credit vouchers will not be offered due to inclement weather cancellations.
