By submitting this form, I confirm that I have read and agree to the following:
Deposit – In order to hold my place in the program, I will submit a $200 deposit to the Office of Global Education and Study Abroad within 3 business days of submitting this application online.The office is located in the Herr House Building--Rooms 207, 208 and 210 on the Parkville campus. Payment may be made in person or by telephone at (816) 584-6830.Please contact us if you need to request alternate payment arrangements.
Meetings – I will attend all pre-trip, on-site, and post-trip meetings and activities. Exceptions to this may be granted on a case by case basis by the trip leader and only for extenuating circumstances.
Liability - I will behave in ways that are responsible, mature and representative of the best of Park University. This means compliance with all applicable state, federal, and international laws as well as the Park University Code of Conduct. This includes the no-alcohol policy during university sponsored events. Ignorance of a law is not an excuse or defense. If your behavior is unacceptable or in violation of this agreement, the trip leader (in consultation with trip coordinators) may terminate your participation in this trip. Termination means you must find your own way home at your own expense (a driver will escort you to the nearest transportation terminal), your lodging and reservations will be canceled, and you will not receive a refund of any of your trip fees.
Participation – I will initiate and participate in activities that help build community, support learning and allow for personal growth. This may include organizing community meals, leading reflections (i.e., group journals, and discussions), planning games or outings, etc. Stick with the group. Any activities done outside of the structured trip activities are at your own risk.
Insurance – Health insurance is required. I will show proof of personal health insurance.
Payment - Final payment is due no later than thirty days before departure. Failure to pay in full by the deadline will result in a cancellation of my application's acceptance.
Cancellation - The deposit and program fees are not refundable.
Agreeing to the above affirmatively indicates my agreement and understanding of the responsibilities I am undertaking as a trip participant. I also personally assume any risks involved in activities outside of the trip program. Submitting this application indicates that I have carefully read and agree to the above conditions.
I affirm that the information given in this agreement is true and correct to the best of my knowledge. I approve contact with the advisor or faculty member named above. I agree to allow the Office of Global Education and Study Abroad access to academic and financial records available through Park University, and authorize the Office of Financial Aid to share information as it relates to the alternative break abroad program.
I understand that the trip fee is a commitment upon submission of application. Upon submission of this form, I understand that I am responsible for all expenses paid on my behalf by Park University, and will repay any such charges if I cancel my trip participation.
