ATTENTION Coaches: Please see the below CHECKLIST to ensure that you have all necessary documents for your team. Please bring this completed form with you during registration.
(TEAM ROSTER)- Complete with ALL participating coaches and players names.
In order to participate, each team must bring each of the below documents for every player listed on their roster.
(TEAM PAYMENT)- Should accompany roster. With total balance reflecting roster fee of $50/player, less $100 non-refundable registration fee paid in advance.
(COACHES WAIVER)- Completed and SIGNED.
(PARTICIPANT WAIVER)- Completed and SIGNED. If minor, (under 18), the waiver must have signature of both participant AND parent/guardian.
(INSURANCE)- Please refer to waiver form for completion of insurance verification. Each participant MUST have valid insurance.
(SPORTS PHYSICAL)-Copy of your most recent/current sports physical. (Please refer to GHSA rules for expired physical dates.)
***No participation will be allowed by a player unless ALL of the above listed these documents are received for that player. This will not exempt overall team participation however.
