International Student's Leave Request Form
If a student decides to go home for a personal reason, the I-20 must be terminated and a new I-20 created along with payment of the $350 I-901 government fee. The I20 may only be signed under certain circumstances and you will need to speak with student services for approval.

Your Visa MUST be valid at your return to Epic, if you do not have a current Visa with a minimum of 1 month prior to expiration you will have to apply again for a Visa with a new I-20 at your embassy.
