Rabies Vaccination PDF*
All applicants must provide a copy of the Rabies Vaccination Certificate. Providing only a receipt will not be accepted.
Spay/Neuter Certificate
Applicant must provide a Sterility Certificate if dog is spayed or neutered. Applicant is not required to provide Sterility Certificate if your dog is not altered.
Once your information is verified, the city will contact you for payment. Please periodically check your email for further instructions. After payment is collected a copy of your issued license will be emailed and a hard copy mailed.
