Leave of Absence Request Form

Use this form to request a leave of absence from Park City Rotary. A leave of absence is only permitted if all outstanding invoices are paid in full, and is granted for a minimum length of 2 months and maximum of 12 months. During this time a Rotarian must pay the monthly Rotary dues and fees but will be excused from paying for lunches and meeting attendance requirements.
