Informed Consent and Acknowledgement I hereby give my approval for my participation in any and all activities prepared by South County Golf Academy and Arroyo Trabuco Golf Club during the selected class. In exchange for the acceptance of myself and my candidacy by South County Golf Academy and Arroyo Trabuco Golf Club. I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless South County Golf Academy and Arroyo Trabuco Golf Club and all its respective officers, agents, and representatives from any and all liability for injuries to myself arising out of traveling to, participating in, or returning from selected class sessions. In case of injury to myself, I hereby waive all claims against South County Golf Academy and Arroyo Trabuco Golf Club including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including golf. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.

Confirmation BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.

Cancellation Policy:

If you need to cancel your class attendance, you will be refunded the full purchase price if you inform us 2 weeks before the start date, minus a $20 processing fee. If you inform us inside of 1 week notice, there will be 50% refunded. If you decide to cancel, or not attend class sessions during the current class session, there is NO REFUND.
