Manager Rebate Request

MANAGERS ONLY - Please include only your managing information. The Rebate Program does not apply to assistant coaches.

Equipment must be returned before rebate can be processed.
Rebate form must be submitted within 30 days of last game before a late application fee of $20/month starts to apply.

Your rebate can be by Zelle or registration credit (1 year to use). Rebate is for up-to $200 of regular registration fees not including any fundraisers or other fees paid during registration. Any deductions made will be from that initial amount.
