MANAGERS ONLY - Please include only your managing information. The Rebate Program does not apply to assistant coaches.
- Equipment must be returned before rebate can be processed.
- Rebate form must be submitted within 30 days of your divisions last game before a late application fee of $20/month starts to apply.
Zelle or Registration Credit (good for next two seasons)

Your rebate can be by Zelle or registration credit (must be used within next two seasons). Rebate is for up-to $200 of regular registration fees not including any fundraisers or other fees paid during registration (including late-registration fee). Any deductions made will be from that initial $200 limit.

REGISTRATION CREDIT INSTRUCTIONS:
1. Upon completion of this form you will receive a code on the 'Thank-You' page. Use this code when registering the child of the team you received the rebate for managing. Enter the code in the Discount/Coupon box on the payment page of the registration.
2. Sometime after registration is completed we will calculate the rebate amount and apply it to the registration.
3. Once rebate credit has been applied we will email you a payment request with any balance still due.
4. Please watch for this email and make payment within one week of receiving that email, or by Player Assessments date, whichever comes first. This email will go to the address you used for the player registration.
