MPB/EVB Parent Draft Lock/Freeze Authorization Form

This form is to ensure that you know and agree to your child being "locked" on a specific coaches team. Please complete this form once for each player in question. Managers should also complete this form for their own children, as it makes it easier for us to know who we're looking for on the list. MANAGERS, you should also have filled out your own teams Lock/Freeze form. Ask your Division Director if you have any questions.

Completing this form does NOT mean your child is guaranteed to be in this coaches team, it is just one step in the requirement process. The manager also has to notify us and is subject to limitations.

VERY IMPORTANT!

Player must be registered and paid in the division of the manager/team that you are approving here. Being registered in the wrong division, unless you did a Request To Change Division Form AND were approved for it, or not being registered at all, or having an unpaid registration will disqualify you from being a "lock". Being on the waiting list also disqualifies.

Completing this form does not guarantee your child a spot in our league, in any particular division or on any particular team, you must still register and pay before the division is full.
