I am registering my child(ren) for the Sew Beautiful Program. I am uploading proof of my hardship and I will bring other required docs with me during the personal orientation. (We will send you an email to let you know what documents you need to bring).I am requesting shoes for my child(ren) and I will upload proof of my hardship such as unemployment, eviction notice, delinquent bills or other justification.
You must upload PROOF of your request as noted above. Black out parents SSN. Do not black out the last four SSN for the children. Do not send any health related documents. Your application will not be reviewed without proper supporting documentation.

If photos, videos and/or recordings are taken of my children or me, I give permission for 2000 Feet, Inc., and its affiliates to use any photos, videos and/or other recordings to be used in a promotional or marketing capacity for 2000 Feet, Inc. There is NO compensation due to me or my child(ren) for the above use.
